110-300-0210(1)(2) Licensor reviewed child files and observed 2 children did not have a completed CIS form. 110-300-0210(3) Licensor reviewed child files and observed the children that did not have a current CIS form did not have a signed and dated statement detailing when the child’s immunizations will be brought up to date and stating they understand their child will be excluded from care if the immunizations are not completed within 30 days of the specified dude date.
